The Provider Information page allows you to perform the following tasks:
· Viewing Provider Information
· Updating Provider Information
· Printing Provider Information
To access the provider information page:
1. Click the Open link next to Provider Information in the EBTProvider Services Tab page. The Provider Information page displays.
The page displays the following tabs, fields, buttons, and links:
· Home
· Services
· News
The page has the following sections of fields:
The following fields display in this section:
o Name
o City
o State
o Phone #
o Fax #
§ Monday
§ Tuesday
§ Thursday
§ Friday
§ Saturday
§ Sunday
o (Required) Transaction Processing Cutoff Time
o (Required) First Name
o (Required) Last Name
· Update

The Update button displays only with the required permission(s), otherwise the child care provider information is read-only and can not be updated. |
